Please follow these instructions when dropping off your Notebook to Service Provider :

  1. 1.     Print this form before dropping / shipping your Notebook off at the (Service Provider).
  2. 2.     Write detailed description of the problem(s) (preferably typed) and include it with the Notebook.
  3. 3.     Back up all data from your hard drive. California Computer is not responsible for any loss of data.
  4. 4.     California Computer will not guarantee any lost or missing components.
  5. 5.     The SP address is listed below for your reference on where to drop off / Shipping your unit for Service Provider.
